Mandatory Quarantine Guidelines
As part of Guam’s ongoing effort to contain the spread of COVID-19, all individuals entering Guam by air or sea shall be subject to a 10-day quarantine, except as provided herein. Arriving passengers must quarantine at a Government of Guam Quarantine Facility (QFAC) unless the individual qualified for quarantine at a rental lodging or personal residence as authorized by the Guam Department of Public Health and Social Services (DPHSS). Individuals may also be eligible for exemption from quarantine provided they present upon arrival the required documents pursuant to this Guidance Memorandum. The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
Government quarantine facilities, and all services included therein, are provided at no cost to travelers.
In coordination with the A.B. Won Pat International Airport Authority, Guam (GIAA), transiting passengers will remain in a designated section of the airport facility while awaiting connecting flights less than 13 hours from landing time to departure time, unless the individual qualifies for exemption from quarantine. Transiting passengers awaiting connecting flights in excess of 13 hours will quarantine at the QFAC until they are transported to the GIAA to transit off Guam, unless the individual qualifies for exemption from quarantine.
All persons exiting the GIAA are required to fill out a Health Declaration form prior to or upon arrival in Guam. Individuals subject to Government of Guam quarantine restrictions are requested to voluntarily comply with the Government of Guam quarantine requirements and indicate their willingness by signing a Voluntary Quarantine Acknowledgement form. Individuals who choose not to sign or do not agree to voluntarily comply with quarantine will be subject to Involuntary Quarantine and issued a Directive for Temporary Quarantine. Individuals who are involuntarily quarantined will also be issued a copy of the 10 GCA Chapter 19 Article 6 and Definitions and notified of their right to legal representation.
Voluntary quarantine regardless of location is subject to monitoring, verification, and enforcement. The knowing and intentional failure to follow any part of this order constitutes a misdemeanor punishable by a fine of not more than $1,000 or imprisonment of not more than one year or both (10 GCA, Chapter 19, §19604 (c) Cooperation).
Travelers arriving into the United States from any foreign country are subject to the Centers for Disease Control and Prevention (CDC) requirement of obtaining a negative pre-departure COVID-19 test result or documentation of recovery from COVID-19, and are still required to undergo quarantine based on their traveler status as defined in this document, subject to exemptions as provided herein.
This guidance does not apply to individuals identified as close contacts to persons with COVID-19. Close contacts will be subject to quarantine based on applicable Guam law, DPHSS case investigation and quarantine protocols, and CDC guidelines, which may extend the quarantine period beyond the time prescribed herein. Travelers who test positive during their quarantine period or while transiting through Guam are subject to DPHSS isolation protocols and are not subject to this guidance.
For questions or concerns regarding this document, please submit your inquiry on dphss.guam.gov/quarantine.
I. ARRIVAL PROTOCOLS
Individuals who are fully vaccinated may be exempt from quarantine. Individuals are considered fully vaccinated 2 weeks after their second dose in a 2-dose series, or 2 weeks after a single-dose vaccine.
Arriving passengers (air or sea) are subject to a 10-day quarantine period, except as provided herein.
By signing a Voluntary Quarantine Acknowledgement form, arriving passengers agree to the following:
1. To comply with the voluntary quarantine order for the period prescribed;
2. To be transported by the Government of Guam directly from the port of entry to the QFAC;
3. To remain in their assigned room while in the designated QFAC;

Transportation will be provided from the QFAC to the GIAA for those traveling by air. However, transportation for individuals departing by sea will be the responsibility of the vessel agent.
Day 5 or Day 6 Test for Release from Quarantine
Individuals in the QFAC may receive, subject to availability and at no cost to travelers, a COVID-19 test on day 5 or day 6 of quarantine, administered by DPHSS or another provider designated by DPHSS.
Acceptable COVID-19 (SARS-CoV-2) tests are defined as (1) molecular tests, such as RT-PCR tests, which detect SARS-CoV-2 genetic material, or (2) antigen tests that detect specific proteins on the surface of the SARS-CoV-2 virus. These tests help to identify whether individuals are currently infected with the coronavirus. At a minimum, test results must include patient name, date of birth, specimen collection date, and the type of COVID-19 test performed.
If the individual receives a negative COVID-19 test result, the individual will be released from quarantine after completion of day 7 but must self-monitor for symptoms of COVID-19 for 14 days after arrival. In the event the individual receives a positive COVID-19 test result, the individual shall be subject to isolation protocols established by DPHSS and shall be isolated at a Government of Guam Isolation Facility.
In the event the individual declines to receive a COVID-19 test while in the QFAC, such individual shall complete the full 10-day quarantine in the QFAC.
